If you need a letter or report from your psychologist or social worker, or if you need a form to be filled-in, please complete and submit the following form. Before you complete this form, please note the following points:
- We will generally need one to two weeks notice to complete letters or reports. Longer or more detailed reports may require even longer.
- There is generally a fee for most letters and reports. Please refer to Our Fees page for information regarding costs.
- Please provide as much information as possible. The more information you provide, the quicker we will be able to prepare your letter or report.
- If you require forms to be filled in, please scan them and upload them using the form below. Alternatively you can drop them in to our office.
